Charity Poker Tournament Registration Form

REGISTRATION CLOSED

Please create a name handle to be used during the tournament, we ask that you please start with your first name initial followed by the first 3 letters of your last name. After that, you can be creative using up to 12 characters. i.e. (Alex Torres = ator@pokerchamp)

By signing (typing my name), I understand that I will be responsible for paying for the rebuys I accumulate during the tournament. I will respond when the organization's represenatives reach out to me for payment.
